A Proper Double Eyelid Surgery that Best Suits Your Eyes

Double Eyelid Surgery by Eye Type

Introduction of various double eyelid surgical procedures that can accommodate various eye shapes and types

Eyes with double eyelids

As for people already having double eyelids, it is important to find a good surgeon since the double eyelid surgery can rather create a strong impression and even make the eyes look smaller.

1. Inner Double Eyelids
Typical inline double eyelids can create a cold impression. After changing the inline double eyelids to the in-outline double eyelids through the procedure, the patient can more comfortably open the eyes and have a soft impression. In this case, incisional double eyelid procedure should be avoided.
2. Relatively Large Eyes
It is very common to see the slightly protruding eyes. They often have indistinct asymmetrical outline double eyelids, and the eyelids are very thin and sensitive. In this case, outline double eyelids are suitable. Also, dual non-incisional technique or partial incisional technique would be the proper choice to create double eyelids without scars.

Thick Eyelids with Excessive Fat

Eyes with thick eyelids can be classified into the protruding eyes with no double eyelids and the thick eyelids with excessive fat or skin.

1. Protruding Eyes with No Double Eyelids
In this case, it is recommended to retain the original shape of the eye rather than completely change the entire shape. This type of eye has a very positive prognosis.

2. Thick Eyelids with Excessive Fat or Skin

Excess Skin above Eyelids

In this case, the skin covering the eyebrow bone is relatively thick and the eyebrow is thick and wide.

Excess Fat above Eyelids

Eyes with excessive fat, but relatively thin skin are usually large in size, and have the soft and sleek texture.

Eyes with Excess Skin and Fat

This is the most complicated case. In this case, the skin is excessively developed so the fat and skin are overly spread. If a patient is young, it would be recommended to wait until she/he loses baby fat. The incisional procedure is recommended.

Eyes with the excessive ROOF

If eyes have excessive fat rather than thick skin, they can be treated by removing fat then creating double eyelids with the dual non-incisional double eyelid surgery or partial incisinal double eyelid surgery. However, if the eyelid skin is thick, double eyelids should be created with the incisional double eyelid surgery or partial incisinal double eyelid surgery.
Especially if eyes have the excessive roof right below the eyebrow bone, double eyelids must be created with the incisional double eyelid surgery. In this case, the ROOF must be properly removed.

Eyes covered with Mongolian Folds that make a dull impression

All mongolian folds might not have to be removed. They should be removed only when they give a dull impression. The presence of the mongolian folds does not mean that the patient requires epicanthoplasty.

1. Epicanthoplasty Needed Case
When you see this picture, you can see how distinctive the eyes have become after epicanthoplasty. Epicanthoplasty is effective for the eye that shows more sclera at the outer corner of the eye than the inner corner or when red flesh at the inner conner of the eye is rarely seen.
2. Considerable Factors When Removing Mongolian Folds
Epicanthoplasty is effective for eyes covered with the mongolian folds that make a dull impression, but the following conditions must be considered.

Asymmetrical Eyes

Most of eyes are not symmetrical from birth but the severity of eye asymmetry significantly varies from completely asymmetry cases to simple asymmetry cases with double eyelid only on one eye. Therefore, it is critical to have a proper understanding about an exact cause of asymmetrical eyes.

1. Simple Eyes Asymmetry with Double Eyelid Only on One Eye
It must be checked whether there are any differences of eyes muscle or it is just differences of double eyelids. If the eyelid pulling power of the eye muscle is significantly asymmetric, it should be corrected via ptosis correction.
2. Difference Size of Double Eyelids
If double eyelids are significantly asymmetric, it is possible to correct both eyes by making average size double eyelids.
3. Complete Asymmetry
Facial asymmetry often developes not only eyes asymmetry but also mouth asymmetry and teeth mis-alignment. The difference strength of eyes muscles can cause a totally different eye shape. In this case, an aesthetic value must be considered first. Even if the asymmetry of the eyes cannot be resolved completely, the procedure should be done if it enhances the appearance.

Protruding Eye

Protruding eyes are relatively common in Koreans. The prognosis of the protruding eyes is usually very positive. However, it should be noted that a surgeon with limited experiences may make a wrong diagnosis and recommend to remove both the skin and fat.

1. Special Features of the Protruding Eye

Having the comparatively large and definite eyes

The eyelid may seem to have excessive fat but it actually does not.

Since the eye tends to easily lose its fat easily, the fat must be removed carefully.

The eyelid is relatively thin.

It does not require epicanthoplasty, but epicanthoplasty could be helpful if the mongolian folds are present.

The eye will be less protrude in beautiful shape.

Eyes with a Wide Distance in Between

For the eyes with a wide distance in between, people immediately believe that epicanthoplasty is essential, but it is not. Of course, epicanthoplasty can be helpful in certain cases, but it also can cause some negative effects as well. Therefore, the patient must have a thorough consultation with a skilled surgeon in advance.

1. Epicanthoplasty Needed Cases
Eyes with a wide distance in between usually are corrected with epicanthoplasty. In case that a distance between the eyes is more than 40mm, 99% can have a positive outcome with epicanthoplasty. However, even if a distance between the eyes is more than 40mm, epicanthoplasty cannot be performed when a distance between the pupils is wide and/or there is almost no mongolian fold.
2. Double Eyelid Surgery Needed Cases
If epicanthoplasty is conducted for the case with divergent strabismus, it could accentuate the divergent strabismus more. In this case, it would be helpful to create the in-out or outline double eyelids.

Droopy Eyelids

Droopy eyelids can make the eyes look smaller and give a strong impression. It is important to select a droopy eyelid correction procedure in consideration of the age and circumstance of the patient. Since the droopy eyes can be caused by various reasons, it is important to analyze an exact cause.

1. Eye that Look Droopy Due to Single Eyelids

This can be correct with non-incisional, dual non-incisional or partial incisional procedure.

2. Eye with Slightly Droopy Eyelids

It can be corrected by making an incision at a relatively higher position.

3. Eye with Severly Droopy Eyelids

It is possible to correct it with non-incisional, dual non-incisional or partial incisional procedure if thin double eyelids suit the patient.

If large double eyelids are required, the incisional procedure is the only option.

4. Patients in Thier 10s~30s who Do Not Have Double Eyelids

In most cases, it is possible to correct the droopy eye without making an incision.

The incisional procedure is required only to completely prevent the unraveling of the double eyelids or when the non-incisional procedure is not available since the skin is severly saggy.

Sleepy-looking Eye

Everyone desires to have a defined and definite eye. Therefore, if you have sleepy-looking eyes, you should correct it with a proper procedure. Sleepy-looking eyes cause analysis is important to plan the operation.

1. Eyes with Severe Wrinkles on the Eyelids
The slightly protruding eye with no double eyelids and indistinct horizontal wrinkles on the upper part of the eyelid right above the pupil is the most common case. In this case, it can be easily corrected almost 100% through a simple procedure.
2. Small Eye
A mono lidded small eye with a little fat and thick eyelid can create a sleepy look. This can be corrected with epicanthoplasty, double eyelid surgery, fat removal and others. Its prognosis is relatively positive.
3. Week Levator Muscle
Ptosis correction is a procedure that corrects the levator muscle, which is responsible for lifting the eyelid. If the eye, which does not require blepharoptosis, is overly corrected with full incisional ptosis correction, it could rather leave only visible scars and induce side-effects. Therefore, the ptosis correction procedure should be conducted only when it is necessary.
4. Unfavorable Position of the Pupil
It is ideal to have the pupil at the center of the sclera and even if not, it can be corrected with double eyelid surgery, epicanthoplasty, and ptosis correction procedure solely or in joint, in consideration of the actual position of the pupil.
