Understanding of Eye Surgical Methods

1. Understanding of Three types of Eye Cosmetic Surgery
Blepharoplasty is usually classified into 3 categories – incisional method, non-incisional method and partial incisional method. It is recommend to have a basic understanding of these methods prior to make a hospital visit.
2. Eye Surgical method Varied By Hospital
It should be noted that the method and design of the blepharoplasty can be varied by hospital and plastic surgeon. Therefore, it is highly recommended to make a visit to a hospital and have a consultation.
3. Finding a Reliable Surgeon
It is more important to find a reliable surgeon with skills rather than focusing on a surgical method. A plastic surgeon should fully explain the ideal objective of a surgery as well as proper surgical techniques, rather than recommend too many surgical procedures.
4. Basic Principles
Patients should comply with basic principles of the plastic surgery, such as not be strict on only one surgical method that is chosen before visit the hospital or believe on what other people said more than the surgeon.

Double Eyelid Shape

The shapes of double eyelids are categorized into three – in, out and in-out. It is important to ask a surgeon about the shape of double eyelids.

1. In
The front part of a double eyelid is not visible, starts to show from the middle. 90% of patients who are willing to have natural-looking eyelids select this shape.
2. Out
The entire double eyelid is visible, starting from the inner corner of the eye, and its thickness stays the same throughout the eye. It is not recommended to a patient with thick eyelid skin or small eyes.
3. In-Out
An entire double eyelid is visible, starting from the inner corner of the eye, but it gets thicker to the outer corner of the eye. It requires a special attention since it could not be as natural as the other two double eyelid shapes.

Double Eyelid Size

As for the size of double eyelids, it is important to consider both sizes of double eyelids when eyes are closed and opened. In general, 7~8mm is considered to be proper.

1. Minimum Double Eyelids

This refers to a very small, but natural-looking double eyelid. The front part is hidden but the rear part is visible.

2. Small Double Eyelids

This is natural-looking double eyelids, which create a feminine look rather than a glamorous image.

3. Medium Double Eyelids

It refers to the double eyelids that can look large after make-up. It is suitable for people with relatively large eyes.

4. Large Double Eyelids

It is not usually a suitable choice for Asians but more proper for the Westerners who have large eyes.

1. Eyelid Condition
2. Shape and Size of the Eye Socket
3. Pupil
Asymmetrical pupils
4. Categorization of Mongolian Folds
5. Angle of the Outer End of the Eye
6. Other factors
7. Recommended Double Eyelid Surgery Type by Eye Condition

Eyes that can easily create a double eyelids line with a sharp pin : Non-incisional technique preferred

Those who have with thin eyelids : Non-incisional technique preferred

Those who are young or reluctant to have a surgical procedure : Non-incisional technique preferred

Droopy eyelids : Incisional technique preferred

Eyes with faded double eyelids : Incisional technique is preferred but also can be done with non-incisional technique if wanted

Eyes with excessive fat : Incisional technique + fat removal, or partial-incisional technique, or incisional technique

Level of Droopiness

Unlike the previous double eyelid surgery for droopy eyelids that focus on removal of excessive skin, it is possible to choose a proper double eyelid surgery in consideration of the position of an eye crease.

1. Double eyelid surgery Type Depending on the Position of Eye Creases
Patients who have small double eyelids formed at the height of 4 mm when eyes are closed and 1mm when opened respectively, with a standard technique, it requires to remove the skin of 2 mm. However, in the advanced double eyelid surgery, suture technique can be made at the height of 8 mm to create the same effect as of the standard technique.

Example of the double eyelid surgery type depending on the position of eye crease

2. The reason why droopy eyes can be corrected with non-incisional double eyelid surgery
If double eyelids are created via non-incision method at the height of 7mm from the eyelash when eyes are closed, it is possible to have the double eyelids at the height of 1~2mm when eyes are opened, creating the same effect as the removal of 3~4mm of the eyelid skin.
Therefore, the double eyelid can be created at the height that can correct the droopy eyelid without cutting off of the skin.
